  You may use this form if you wish to issue a formal complaint against faculty or staff (support and administrative) concerning the areas of alleged discrimination and academic matters, excluding individual grades except under certain conditions. Any student who has an unresolved complaint with a faculty or staff member, other students or student groups, or administrators has the right to file a written complaint without fear of prejudice of his or her college status. Students who are unable to resolve a complaint at an informal conference with the instructor or staff member, or the member’s direct supervisor, may file a written complaint using the form linked in this activity.
